Event Description
It was reported, that the power button on the xenon light source was stuck, and the front bezel was not working.There were no reports of patient harm.
 
Manufacturer Narrative
The device was returned, and the evaluation found a faulty scope socket.Three attempts were performed to obtain additional information, but no response was received from the customer.Should additional relevant information become available, a supplemental report will be submitted.

The issue was found during preparation for use for an unspecified procedure.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, it is likely the following led to the malfunction: power is stuck and front bezel is not working: it is likely that the front bezel does not work due to a failure of the power switch.Faulty scope socket: it is likely that the scope detection is not working due to a failure of the scope socket.However, a conclusive root cause could not be determined.Olympus will continue to monitor field performance for this device.
