An investigation is in progress to determine the cause of this reported event.An return material authorization (rma) was issued to evaluate the intuitive surgical, inc.(isi) device.Additional information is being gathered to determine the contribution of the device to the customer reported issue.
 
Event Description
It was reported that during a da vinci-assisted benign hysterectomy surgical procedure, the force bipolar instrument looked different at hinge tip.When the customer tried to take it out, it was unable to come out of the trocar.The customer had to use the emergency stop and undock arm to remove everything.The instrument side hinge was broken.The procedure was completed with no reported injury.Intuitive surgical, inc.(isi) followed up with the initial reporter and obtained the following additional information: the force bipolar instrument was inspected prior to use, and nothing was noted.When the customer was tried to remove the instrument, it was unable to come out of the trocar.The customer had to use the emergency stop and undock to remove everything.The port incision was not increased in order to remove the instrument and cannula together.There were no fragments fell into the patient.The instrument did not collide with any other instrument or tool during procedure.The force bipolar instrument jaws were not stuck in a closed position prior to attempting to remove.
 
Manufacturer Narrative
Intuitive has received the prograsp forceps instrument associated with this complaint and completed investigations.Failure analysis investigations did not replicate nor confirm the customer reported complaint.Visual inspection did not reveal any damage.The instrument was placed and driven on an in-house system and passed the recognition and engagement tests.The instrument moved intuitively with full range of motion in all directions.The grips opened and closed properly.The grips were aligned.The instrument released energy.The instrument was fully functional.
