It was reported that during a da vinci-assisted benign hysterectomy surgical procedure, the force bipolar instrument looked different at hinge tip.When the customer tried to take it out, it was unable to come out of the trocar.The customer had to use the emergency stop and undock arm to remove everything.The instrument side hinge was broken.The procedure was completed with no reported injury.Intuitive surgical, inc.(isi) followed up with the initial reporter and obtained the following additional information: the force bipolar instrument was inspected prior to use, and nothing was noted.When the customer was tried to remove the instrument, it was unable to come out of the trocar.The customer had to use the emergency stop and undock to remove everything.The port incision was not increased in order to remove the instrument and cannula together.There were no fragments fell into the patient.The instrument did not collide with any other instrument or tool during procedure.The force bipolar instrument jaws were not stuck in a closed position prior to attempting to remove.
