Event Description
|
An aneurx stent graft was implanted during the endovascular treatment of an aaa.It was reported that a follow-up ct taken approximately 17 years post the index procedure showed anterior stent graft fractures just proximal to the flow divider and an endoleak.The physician relined the stent graft with a non-mdt device to exclude the aneurysm and resolve the endoleak.Per the physician the cause fracture was device related post 17 years of being implanted.No additional sequelae were reported and the patients is fine.

Product analysis conclusion: the reported stent fracture and endoleak were confirmed on the films provided; however, the cause of the events could not be conclusively determined.Lack of pre implant cts did not allow for assessment of the pre-implant anatomy, and earlier post-implant cts were not available for comparison of the stent graft in vivo configuration over time.During analysis, likely distal migration with loss of proximal seal were observed.It is possible that disease progression with aneurysm morphology changes over the almost 14 years of device implantation may have contributed to the observed likely migration with proximal loss of seal and stent fractures, but this could not be confirmed.The endoleaks observed in the fractured areas were most likely type iii fabric endoleaks due to stent struct fracture, which likely contributed to tear of the endograft fabric.The cause of the fractures could not be determined.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report.This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
