In speaking with the olympus technical assistance center (tac), the customer reported that they have one tower.All scopes would not physically go into the socket correctly.The tac asked if they could see anything blocking socket holes and the customer stated no.On site support was requested but the tac explained that there are no settings to change that would affect this, and if the socket is physically broken then this would need to be repaired at olympus as these units are not on site repaired.To date, the device has not been returned.Should additional relevant information become available, a supplemental report will be submitted.
 
Event Description
It was reported, the xenon light source would not go into the socket correctly.The customer needed help to attach the scope and set it up.The issue occurred after an unspecified procedure.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Additional information added to fields d8, d9, h3 and h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eleven (11) years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause was not identified; however, the probable cause of the malfunction would likely due defective socket because the locking mechanism and scope detection slide were not working.Olympus will continue to monitor field performance for this device.
