It was reported this was a mitraclip procedure to treat degenerative mitral regurgitation (mr) with a grade of 4 and enlarged atrium.An xtw clip was inserted and grasping was performed.While repositioning and grasping the leaflets again, the posterior gripper would not move.Troubleshooting was performed, but the issue was unable to be resolved.Therefore, the clip was removed and replaced.While outside the anatomy, tissue was observed to be on the clip arm and gripper.The procedure was continued, and two new clips were successfully implanted, reducing mr to a grade of 1.There was no clinically significant delay in the procedure.

All available information was investigated and the reported gripper actuation issue was not confirmed via returned device analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other complaints reported from this lot.The investigation determined the reported gripper actuation issue appears to be related to the reported tissue injury.The reported tissue injury appears to be due to multiple grasping.Tissue injury is listed in the instructions for use is a known possible complication associated with mitraclip procedures.The reported serious injury/illness/impairment was a result of case specific circumstance as no intervention performed to address the tissue injury.There is no indication of a product issue with respect to manufacture, design or labeling.
