B3 date of event: date of event was approximated to 01/01/2024 as no event date was reported.Device evaluated by mfr: the device was returned for analysis.A visual examination of the balloon identified no damages.A hypotube break was identified 24cm distal to the distal end of the strain relief.Multiple kinks were identified along the length of the hypotube shaft.No kinks or damages were identified along the polymer shaft.A detailed microscopic examination of the balloon material identified no tears or pinholes in the balloon.All blades were fully bonded on the balloon and did not exhibit any signs of damage.No issues identified during examination of the extrusion shaft.Tip showed no signs of tip damage.A microscopic examination of the proximal and distal markerbands identified no damage.
 
Event Description
Reportable based on device analysis completed on 28feb2024.It was reported that the wolverine was unable to reach target lesion due to torturous vessel.A 10mmx3.50mm wolverine cutting balloon was selected for percutaneous transluminal coronary angioplasty (ptca).During the procedure, wolverine was unable to reach target lesion due to torturous vessel.The procedure was completed with a different device.No patient complications were reported.However, device analysis revealed that a break had occurred in the hypotube located at 24cm distal to the distal end of the strain relief.
