Event Description
It was observed that during the device evaluation, the ultrasound gastrovideoscope exhibited adhesive peeled off or cracked due to a shock which created a gap in the adhesive on the distal end, and a stain entered inside the lens through the gap.There were no reports of patient involvement.

The device was returned and the evaluation found no malfunctions aside from the allegation reported in b5.Should additional relevant information become available, a supplemental report will be submitted.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Information added to the fields: h4, h6.Correction in the fields: d5, e2, e3.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 8 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause for the event gap between acoustic lens and ultrasound probe could not be determined.Olympus will continue to monitor field performance for this device.
