Event Description
It was observed that during the device evaluation, the gastrointestinal videoscope nozzle was clogged with surgical glue.There were no reports of patient involvement.
 
Manufacturer Narrative
The device was returned, and the evaluation found that due to clogging of nozzle, water supply volume did not meet the standard value.Should additional relevant information become available, a supplemental report will be submitted.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Corrected fields: d9 additional information added to field: h6 please note h3 was inadvertently selected and cannot be removed, disregard this selection as it was already selected on the initial.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the foreign material clogging the nozzle was believed to be surgical glue.It is likely the foreign material remained in the device because reprocessing could not be conducted properly due to a leak from the biopsy channel.However, a definitive root cause could not be determined.The suggested event may be detected and prevented by handling device in accordance with the following ifu.Ifu: gif-ez1500 operation manual chapter 3 preparation and inspection states how to detect the event.Ifu: gif-ez1500 reprocessing manual chapter 5 reprocessing the endoscope states how to prevent the event.Olympus will continue to monitor field performance for this device.
