Event Description
It was reported, that the gastrointestinal videoscope had no secondary water supply.There were no reports of patient harm.

This supplemental report is being submitted to provide the correction of the initial mdr and the legal manufacturer's final investigation.Corrected fields: h6 (missing component code since the initial) additional information: h3 and h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over thirteen (13) years since the subject device was manufactured.The device was returned to olympus for inspection, and the customer's complaint was confirmed.A definitive root cause was not identified, however based on the results of the investigation, the probable cause of the "clogged auxiliary water channel and foreign material in the nozzle¿ malfunctions could not be identified, nor the type of material.The event can be prevented by following the instructions for use which state: ifu states that detection method in evis lucera gif/cf/pcf type 260 series operation manual chapter 3 preparation and inspection.Ifu states that preventive measure in evis lucera gif/cf/pcf/sif type 260 series reprocessing manual chapter 3 cleaning, disinfection, and sterilization procedures.Even though the customer provided some cleaning disinfection and sterilization information, it remains unknown if there were any deviations from ifu in reprocessing steps for the area foreign material remained.Olympus will continue to monitor field performance for this device.
