Event Description
It was reported that the patient's right ventricular lead was replaced during implant due than an unknown malfunction.An alternative lead was implanted on (b)(6) 2024.The patient's condition was unknown.
 
Event Description
Additional information received noted that the right ventricular lead dislodged and could not be implanted during procedure.It was also noted that the helix of the lead felt as if some tissue was stuck in it.
 
Manufacturer Narrative
The reported event of helix mechanism issue was confirmed while the reported event of unable to implant and lead dislodgement were not confirmed.As received, a complete lead was returned in one piece.For analysis.Visual inspection found the helix extended, bent, stretched, and clogged with blood.X-ray examination found the inner coil over-torqued consistent with procedural damage.The cause of helix mechanism issue was isolated to the helix being damaged, clogged with blood, and over-torque of the inner coil.
