Event Description
It was reported the xenon light source had error code b30.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 6 years since the subject device was manufactured.The device was not returned to olympus for inspection, and the customer's complaint was not confirmed.Based on the results of the investigation, a definitive root cause could not be determined.Troubleshooting: the sales representative is informed that the user turns off the clv-190 before connecting or disconnecting the oscilloscope.If b30 appeared, both the cv-190 and clv-190 must be turned off for the error to reset before trying another oscilloscope.I advised the sales representative to advise the customer to clean the contact points of the headset with an alcohol prep pad to remove debris, hair, and fingerprints before connecting it to the clv-190.Double check to see if they have new cleaning staff who may have introduced a liquid invasion into the range.Recheck the vent connector on the sight glass and leak tester for wear.He said the customer will investigate and send the clv-190 for evaluation if necessary.No additional tac assistance is required.Three attempts were made to obtain additional information regarding the reported event, but no response was received from the customer.Olympus will continue to monitor field performance for this device.
