Event Description
The customer reported to olympus the light source had loose receptacle and video connection issues with multiple scopes.The issue was found during preparation for use.There was no report of patient injury or medical intervention associated with this event.
 
Manufacturer Narrative
To date, the device has not been returned to olympus for evaluation.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 3 years since the subject device was manufactured.The device was returned to olympus for inspection, and the customer's reportable malfunction was not confirmed.Based on the results of the investigation, the loose connection and video connection issues experienced across multiple scopes were caused by the loose connection during their setup.However, a definitive root cause could not be determined.Olympus will continue to monitor field performance for this device.
 
Event Description
It was reported that the xenon light source had loose receptacles and video connection issues with multiple scopes.The issue occurred during a colonoscopy procedure that was completed using the same set of equipment.There were no reports of patient harm.
