Event Description
Reportable based on device analysis completed on 07mar2024.It was reported that the part where balloon and shaft was joined got stretched.A 15mmx2.25mm wolverine coronary cutting balloon was selected for use.During the procedure, outside patient's body, there was an abnormality on the appearance before usage as the part where the balloon and shaft was joined got stretched.The procedure was completed with another of the same device.No complications reported and there was no patient injury reported.However, device analysis revealed that the inner wire lumen broke 5cm from the tip of the device.
 
Manufacturer Narrative
Device evaluated by mfr: no issues were noted with the hypotube shaft.The inner wire lumen was broken 5cm from tip of the device with excessive stretching along the distal extrusion.A detailed microscopic examination of the balloon material identified no tears or pinholes in the balloon.All blades were fully bonded on the balloon and did not exhibit any signs of damage.The inner wire lumen was broken 5cm from tip of the device with excessive stretching along the distal extrusion.Tip showed no signs of tip damage.A microscopic examination of the proximal and distal markerbands identified no damage.No other issues were identified during the product analysis.
