Event Description
It was reported that a mitraclip procedure was performed to treat a functional mitral regurgitation (mr) with grade of 4.During the first establish final arm angle (efaa) step, the clip was observed to continuously open.It was indicated that when performing efaa, an audible click was heard.The clip arms were closed, and standard troubleshooting was performed.However, the clip was observed to continuously open during the second efaa.The clip was removed, and a replacement was used to complete the procedure.One clip was implanted, reducing mr to grade 1.There was no clinically significant delay in the procedure and no adverse patient effects.
 
Manufacturer Narrative
All available information was investigated, and the reported unintended movement (clip open ¿ efaa) and noise were not confirmed via returned device analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history did not indicate a lot-specific product issue.Based on the information provided and the results of the returned device analysis (unable to confirm the reported issues), a cause for the reported unintended movement (clip opened while establishing final arm angle) and noise-audible noise could not be determined.There is no indication of a product issue with respect to manufacture, design, or labeling.
