Event Description
It was reported that the device turns on and off while using the insufflation unit.The issue was found during a therapeutic robot assisted laparoscopic prostatectomy with a 30-minute delay.There was no patient harm associated with the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and device evaluation.The device was evaluated by olympus, and the following was observed: the device generated an alarm, and the front panel did not display due to error code e03 caused by a defective main board.It was also confirmed that the device power turned on/off automatically and repeatedly due to the faulty power supply.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been more than 8 years since the subject device was manufactured based on the results of the investigation, it is likely that the device powering on/off (which led to the 30-minute procedural delay) was due to a failure of the power supply.The displayed error code (e03), device alarm, and front display issue was likely due to a defective main board.However, the root cause of the event could not be determined.This supplemental report is being submitted to provide a correction to the initial medwatch.The reportability decision was inadvertently submitted as a product problem only.However, this complaint has been determined to also meet the criteria of a serious injury due to the reported 30-minute procedural delay.Also, a correction has been made to g2 to provide information that was inadvertently not included in the initial medwatch.Olympus will continue to monitor field performance for this device.
