Event Description
It was reported this was a mitraclip procedure to treat degenerative mitral regurgitation (mr) with a grade of 4+, flail, and a small atrium.Two clips were successfully implanted, reducing the mr to a grade of 2-3.While retracing the clip delivery system (cds) into the steerable guide catheter (sgc), it was observed the tip of the cds caused damage to the atrial wall.Which resulted in a pericardial effusion.Physician performed pericardiocentesis to treat the pericardial effusion.There was no clinically significant delay in the procedure.

The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.The investigation determined the reported perforation appears to be related to procedural condition and the pericardial appears was a cascading effect of the perforation.Perforation and pericardial effusion are listed in the instructions for use as known possible complications associated with the mitraclip procedures.The reported unexpected medical intervention was a result of case specific circumstance as pericardiocentesis was performed to treat the pericardial effusion.There is no indication of a product issue with respect to manufacture, design or labeling.
