|
A customer reported that during a surgery an ophthalmic system displayed error messages and turned off twice on its own.As a result, there was a temporary anterior chamber collapse in the patient, with no lasting damage observed.Additionally, the footswitch cable was found to be damaged.Additional information has been requested but none received till date.This complaint is pertaining the one of two reports received from the initial reporter.

Event Description
|
Per additional information received, during phacoemulsification surgery on the right eye, when removing the quadrants collapse of the anterior chamber resulted in aspiration of posterior capsule with defective capsule.That resulted in additional surgical intervention anterior vitrectomy procedure and sulcus-fixated intraocular lens (iol).
