Event Description
It was reported the light source light-emitting diode on the front panel flash constantly, even after switching on/off it remains so permanently.The issue occurred during preparation for use.There were no reports of patient harm.

The device evaluation is ongoing.Should additional relevant information become available, a supplemental report will be submitted.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Since the device is more than 15 years old, the device history record was unable to be reviewed.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.The device was returned to olympus for inspection, and the customer's complaint of all light-emitting diode on the front panel flash constantly was confirmed.Based on the results of the investigation, it is likely the turret motor failure and the effect of damage caused by fatigue due to repeated operation or malfunction due to stuck of motor led to the malfunction.In addition, the technical evaluation report confirmed that the defect reported by the customer is the result of worn out motors.Olympus will continue to monitor field performance for this device.
