Event Description
Philips received a complaint from the customer on the v60 ventilator indicating that the device was reading high o2 pressure and indicating oxygen was not available.It was reported there was no patient involvement at the time the issue was discovered.There was no report of patient or user harm.The remote service engineer (rse) and customer troubleshot the device for the reported issue.It was discovered that the device was originally attached to a wall oxygen (o2).An o2 tank was then attached, and the wall o2 was removed.This created a buildup of pressure in the manifold and did not allow the device to switch to tank o2.The customer allowed the o2 line to release and confirmed that the device started functioning properly.The v60 user manual includes the instruction to make sure the cylinder regulators are turned off while the ventilator is connected to wall oxygen.The customer released the pressure buildup in the manifold to resolve the reported issue.The device passed the required performance verification tests per philips standards and was returned to service.
