Event Description
It was observed during the device inspection, that the flex deflectable videoscope exhibited adhesive peeling of the objective lens tip and a cloudy image.There were no reports of patient involvement.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and device evaluation.The device was returned to olympus for inspection, and we could not specify the root cause of the peeled adhesive, however, it is probable that the defect was cause by external factors or handling.Additionally, we could not specify the root cause of the foggy image, however, it is probable that moisture entered the endoscope due to the air leak.We were unable to reproduce the reported issue.A review of the device history record found no deviations that could have caused or contributed to the reported issue.The event can be detected by following the instructions for use: "chapter 3: preparation and inspection, item 3: inspection of the endoscope" in the instruction manual (operation section).Olympus will continue to monitor field performance for this device.
