It was reported the oer-pro buttons were not working.The process could not proceed even when the confirm button was pressed after replacing the water filter.The issue occurred during reprocessing.There were no reports of patient harm.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over five (5) years since the subject device was manufactured.The device was not returned to olympus for inspection, therefore the customer's reportable malfunction could not be confirmed.Based on the results of the investigation, a definitive root cause could not be determined.However, it is likely that the process does not proceed even if the confirm button is pressed after replacing the water filter occurred due to the integrated chip in main control panel/sub control panel had failure by accumulated stress by use of the equipment or accidental factor to cause.Olympus will continue to monitor field performance for this device.
