Event Description
"deployed the stay-behind sheath into the eia to deploy the leg into the eia.While the clinician was inserting a catheter to do a run to identify the iia the sheath moved back and came out of the eia, causing significant bleeding and the sheath having to replaced by an alternative device.The clinician felt that the reason the sheath came out was because the radiopaque marker can't be seen under fluoroscopy.An e-mail received on (b)(6) 2024 from adrian lancaster, regional sales manager uk north, stated that: "there are no images to support this complaint"." patient outcome - "procedure was completed successfully.".
