Event Description
|
It was reported that the male luer connector at the end of a y-type blood/soln set broke; the "rotating securement cap" broke from the tubing, causing the connector sleeve to move up and down the line.The inner component of the luer connector was stuck in the needleless port component of the non-baxter tubing.This occurred during patient use, before the blood product was spiked.A new line was used to continue treatment.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

H11: the actual device was received for evaluation.Visual inspection was performed which identified the two piece luer lock assembly was broken.A functional test was performed, and no leaks or blockages were noted.The reported condition was verified.The cause of the condition could not be determined; however, a probable cause is related to a force generated by the end user while trying to disconnect the sets.Should additional relevant information become available, a supplemental report will be submitted.
