Event Description
It was reported that during the implant procedure, the right atrial (ra) lead was placed at the ra appendage.The lead fell off after the helix was screwed out.The lead could not be fixated despite multiple attempts.The ra lead was attempted, not used and was replaced.No patient complications have been reported as a result of this event.
