Event Description
It was reported, the xenon light source did not power on.There were no reports of patient harm.

This report is being supplemented to correct information provided in the initial medwatch report and to provide additional information, based on the legal manufacturer's final investigation and device evaluation.The device was returned to olympus for inspection.And the customer's complaint was confirmed.The unit did not turn on, because the lamp door was broken.And it could not be attached to the top cover.A review of the device history record found no deviations, that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation, it is likely, that the device did not turn on, because the lamp door was broken.And it could not be attached to the top cover.However, a conclusive root cause could not be determined.Olympus will continue to monitor field performance for this device.
