Event Description
It was reported that the guidewire lumen detached from the tip of the catheter.During an ablation procedure to treat atrial fibrillation a farawave pulsed field ablation catheter was selected for use.After successfully ablating the left superior pulmonary vein (lspv) the catheter was moved in the flower shape to the left inferior pulmonary vein (lipv).When the catheter array was switched to the basket configuration it could no longer deploy.When the catheter was removed from the patient it was found that the guidewire lumen had detached from the tip of the catheter.The procedure was completed with another farawave catheter.No patient complications were reported.The device is expected to be returned for analysis.
