Event Description
It was reported that device sterility was lost.The permanent sled sterile bag was used as part of an intravascular ultrasound (ivus) examination of the target lesion.During the procedure, the sterile bag was opened, and the motor drive unit was inserted into the bag.However, the plastic hub over the catheter port tore as the staff member was trying to pop it into place, resulting in lost sterility.The procedure was completed with another of the same device.There were no patient complications reported.
