Event Description
It was reported during a pulse field ablation (pfa) procedure using a farawave pfa catheter the guidewire became stuck in the catheter.When the catheter was deployed at the left superior pulmonary vein (lspv) the physician found the guidewire was stuck and the catheter's slide switch could not be moved.The physician continued to manipulate the slider switch and eventually the guidewire came unstuck.The procedure was completed using the original catheter with no patient complications.The catheter is not expected to be returned as the facility reported the device as lost.
 
Manufacturer Narrative
It was indicated that the device will not be returned for evaluation as the facility reported it as lost.If there is any further relevant information obtained, a supplemental medwatch will be filed.
