Event Description
|
It was reported that a small undetectable hole was in the venous line of a prismaflex tpe2000 set during continuous renal replacement therapy, which allowed air to fill the circuit.The extracorporeal blood return was initiated however the treatment was terminated due to clotting in the filter.There was no report of patient injury associated with this event.No additional information is available.

H11: the actual device was not available; however, photographs of the sample were provided for evaluation.Visual inspection of the photos showed a small mark on the return line with no visible hole.A hole in the tubing would have allowed for an external fluid leak versus an air inlet due to overpressure in the return line.The observed mark has no link to the reported presence of air in the line.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.The reported condition was verified.The cause of the condition was not determined as no further or actual sample testing could be performed.Should additional relevant information become available, a supplemental report will be submitted.
