It was reported that two (2) large volume infusors were leaking from the bladder.These issues were described as, ¿ball inside the bottle that leaks, and the bottle starts to have liquid after the liquid is filled¿.The leaks were discovered in the pharmacy before patient use.There was no patient involvement.No additional information is available.

Additional information was added to d4: expiration date, d9.H4: the lot was manufactured between june 28, 2023 ¿ june 30, 2023.H11: two (2) devices were received for evaluation.A visual inspection was performed, and a pool of fluid inside the cover was identified which suggests a leak has occurred.A leak test was performed, and a leak was observed coming out at one side of the bladder crimp.The reported condition was verified.The cause of the condition was related to manufacturing.A nonconformance has been opened to address this issue.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
