The reported complaint that the autopulse platform (b)(6) displayed user advisory (ua) 07 (discrepancy between load 1 and load 2 too large) was confirmed during functional testing and review of the archive data.The root cause of the (ua) 07 was due to a failure of single point load cell #2, likely attributed to failed component(s), or mishandling such as a drop.Visual inspection of the returned platform was performed, and no physical damage was observed.A review of the archive data showed (ua) 07 messages; thus, confirming the reported complaint.The autopulse platform failed the initial functional testing due to the user advisory (ua) 07 error message displayed upon powering on.The load cell characterization test indicated load cell # 2 was over-reporting.Single point load cell #2 will be replaced to address the observed failure.Upon service completion, the platform will be subjected to final functional testing to ensure that the device passes all testing criteria.Historical complaints were reviewed for service information related to the reported complaint, and there were no similar complaints reported for autopulse platform with (b)(6).
 
Event Description
The customer reported during a training session, the autopulse platform (b)(6) displayed user advisory (ua) 07 (discrepancy between load 1 and load 2 too large).Per the customer, the training occurred a couple days after it was used on a patient where it worked perfectly.In between these two uses it was not used, dropped or damaged in any way.No patient involvement.
