It was reported, the light guide cable adapter of xenon light source was inserted alone and could not be removed.There were no reports of patient harm.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Corrected data: h6 component code.Additional information added to field b3, b5, h3, h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 8 years since the subject device was manufactured.The device was returned to olympus for inspection, and the customer's allegation was confirmed.Based on the results of the investigation, it is likely the phenomenon was caused because the light guide cable adapter was inserted alone.Olympus will continue to monitor field performance for this device.
