Event Description
It was observed that during the device inspection, the bronchovideoscope connecting tube coating was observed to be peeling.There were no reports of patient involvement.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Corrected fields: d5 and h6.Additional information added to field h3 and h6.A review of device history record and historical complaints analysis was conducted and no deviations that could have caused or contributed to the reported issue were identified.Based on the results of the investigation, it is likely that the coating of the insertion tube was peeled off due to the physical or chemical stress applied to insertion section.However, the root cause could not be established.The following is included in the instructions for use (ifu): 3.3 inspection of the endoscope: 3 inspect the external surface of the entire insertion section, including the bending section and the distal end for dents, bulges, swelling, scratches, peeling of coating, holes, sagging, transformation, bends, adhesion of foreign bodies, missing parts, protruding objects, or other irregularities." olympus will continue to monitor field performance for this device.
