Event Description
It was reported that during the stage two implant procedure, the physician had a hard time removing the deep brain stimulation (dbs) lead cap.The lead would not come out, therefore, the physician had to cut through the cap while the metal was still attached and cannot be removed.The physician tried to insert the lead into the extension, but was not successful as the lead was bent and damaged.The physician had to cut the lead in order to finish the procedure.The patient later underwent a revision procedure where the right lead was explanted and replaced with a new lead.The hospital retained the explanted lead.The patient was doing well postoperatively and has fully recovered.
