Event Description
It was observed during the device inspection, that the bronchovideoscope exhibited a burned distal end plastic cover and the image guide tube indication was missing.There were no reports of patient involvement.

The device evaluation is ongoing.Should additional relevant information become available, a supplemental report will be submitted.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 4 years since the subject device was manufactured.Based on the results of the investigation, it is likely that the distal end cover burnt was the result of using a high-energy instrument (laser, radiofrequency, etc.) without maintaining a sufficient distance from the distal end.And the marking disappearance on the insertion tube was likely caused by external factors, or handling.However, olympus could not identify a definitive root cause of the event.Olympus will continue to monitor field performance for this device.
