The technical assistance center performed troubleshooting with the customer, and it was advised that it must be soaked for up to 10 hours, or until debris is loose, and then manually cleaned.The evaluation of the event is ongoing.Should additional relevant information become available, a supplemental report will be submitted.

It was reported that the uretero- reno videoscope was not cleaned immediately after use.There were no reports of patient harm.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the device was not cleaned immediately after use, considering that the user¿s understanding of device handling and reprocessing steps was different from the recommendation by olympus.However, a definitive root cause could not be identified.The instruction manual states the preventative measures in "urf-v3 reprocessing manual chapter 5 reprocessing the endoscope".Olympus will continue to monitor field performance for this device.
