Event Description
Of blood.The reporter stated that the spike was not attached very well to the chamber and the spike fell off completely when changing out the set.The event was further explained as a tiny leak during infusion from the spot where one of the two spikes was connected to the tubing.The infusion was stopped, and during the set change the spike came apart from the tubing.There was no patient injury or medical intervention associated with this event.Additional information was requested and is not available.

(b)(4).A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.The sample was not available for evaluation as it was discarded by the customer.Should additional relevant information become available, a supplemental report will be submitted.
