It was reported to boston scientific corporation that an rx cytology brush was used in the common bile duct during an endoscopic retrograde cholangiopancreatography (ercp) procedure performed on (b)(6) 2014.According to the complainant, during the procedure, the user moved the brush in and out of the stricture when it eventually separate from the handle resulting to difficulty actuating the device.No part of the device that detached was left inside the patient.It was also reported that the brush would not retract back into the catheter so they removed the device by pulling the catheter out together with the brush.No damaged was noted with the device or its packaging prior to use.The procedure was completed with this rx cytology brush.There were no patient complications reported as a result of this event.The patient's condition at the conclusion of the procedure was reported to be fine.

Patient's exact age is unknown; however, it was reported that the patient was under the age of 18.Reported of event drive wire detached.The device has not been received for analysis.Upon receipt and completion of the failure analysis of the complaint device, if there is any further relevant information from that review, a supplemental mdr will be filed.
