The patient was in the garden and when she was finished her back was tender, she had a bump/knot at the top of her incision (felt like something was there).The patient had pain, redness and swelling at the lead location.Impedance checks were good.X-rays were taken and compared to procedural x-rays.One of the leads migrated and pulled down 4 vertebral bodies and was now sitting at the top of the patient¿s incision at the spot she was complaining of pain.There was an insertion retention issue with the anchor.The patient was not using the device.A revision was going to be scheduled when the patient¿s surgeon gets back in town.Additional information has been requested.
