Event Description
|
It was reported to boston scientific corporation that an rx cytology brush was used in the common bile duct during an endoscopic retrograde cholangiopancreatography (ercp) procedure performed on (b)(6) 2014.According to the complainant, during preparation, the tip of the brush was kinked.The procedure was completed with another rx cytology brush.There were no patient complications reported as a result of this event.The patient's condition at the conclusion of the procedure was reported to be stable.

Patient's exact age is unknown; however, it was reported that the patient was over the age of 18.Reported event of brush bent.The device has not been received for analysis.Upon receipt and completion of the failure analysis of the complaint device, if there is any further relevant information from that review, a supplemental mdr will be filed.

Investigation results: visual evaluation of the returned device found bristled portion of the brush to be bent on an angle with no visible residue.Functional evaluation revealed device to be extended to about 4.5cm but was unable to fully retract.The complaint that the brush bent was confirmed, brush was found to be bent and was unable to retract due to the device condition.The failure was noted during preparation and while outside patient.Therefore, the most probable root cause for this event is determined to be ¿handling damage¿.A review of the device history record (dhr) confirmed that the device met all material, assembly, and product specifications at the time of release to distribution.A search of the complaint database confirmed that no other complaints exist for the specified batch.
