Event Description
It was reported that during an advance sling implantation surgery, the physician "cut off sling arms, he mistakenly left one of the plastic sheaths on one of the arms (patient's left)." the physician "realized what he did and tried to remove sheath." he was not able to find the sheath so he left it after "long attempt." the sling was implanted successfully however, the surgery was prolonged in an effort to remove the sheath.No patient complications have been reported in relation to this event.
