Event Description
It was reported to boston scientific that a polaris loop ureteral stent was used during a stent placement procedure on (b)(6) 2014.According to the complainant, during follow up it was noticed that two loops on the stent became broken, but not detached from the stent.It was reported that during the procedure the suture was cut and removed from the stent.There has been no reported deficiencies regarding the functionality of the stent and the stent currently remains implanted inside the patient.There were no patient complications reported as a result of this event.The patient's condition at the conclusion of the procedure was reported to be good.
 
Manufacturer Narrative
Reported event of stent loop was torn.The complainant was unable to provide the suspect device lot number; therefore, the lot expiration and device manufacture dates are unknown.However, the complainant reported that the device was not expired.The device has not been received for analysis.Upon receipt and completion of the failure analysis of the complaint device, if there is any further relevant information from that review, a supplemental medwatch will be filed.
