Event Description
The customer reported that their device appeared locked up by showing a partial solid white screen with vertical lines across the screen.The device likely could not function in this state.There was no patient use associated with the reported issue.

(b)(4).Physio-control continues to investigate the reported failure and will submit a supplemental report on this event to the fda as provided by 21 cfr 803.56.
 
Manufacturer Narrative
Physio-control evaluated the customer's device and verified the reported issue.Physio then replaced both the system controller (sc) and user interface (ui) pcb assemblies and completed other, unrelated, repairs.After observing proper device operation through functional and performance testing the unit was returned to the customer for use.Physio further examined the removed ui pcb assembly and determined that the cause of the reported issue was an electrically shorted integrated circuit (ic) chip, designator u8.The sc pcb assembly was an ancillary part and there was no failure of the assembly.
