It was reported that during set-up of the device for a cardiopulmonary bypass procedure, the centrifugal controller unit display failed.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.

The reported complaint was verified by the field representative (fsr).The fsr disassembled the centrifugal control unit.The sr found the ribbon cable that goes from the display board to the pd board was disconnected.The customer has the unit pole mounted but not tightened down.The unit is loose enough where they can swivel it around.The unit most likely got jarred which probably caused the issue.The latch down tabs on either side of the cable connection were loose.When reinstalling the cable, the fsr put silicone sealant on each side latch.The fsr reassembled the unit, the display came on with no further issues.The fsr tested the unit.The unit operated to manufacturer specifications and was returned to clinical use.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
