Event Description
It was reported that "while the surgeon was inserting the cage on the inserter, there was no audible ratcheting sound as the device was expanded.He also stated that when the device was placed in situ and the inserter removed, the cage collapsed rather than maintaining the desired level of correction.".

(b)(4): neither the device nor films of applicable imaging studies were returned to the manufacturer for evaluation.Therefore, we are unable to determine the definitive cause of the reported event.
