Event Description
|
It was reported to boston scientific corporation that an rx cytology brush was used in the common bile duct during an endoscopic retrograde cholangiopancreatography (ercp) procedure performed on (b)(6), 2015.According to the complainant, during unpacking, the bristled portion of the brush was already bent.The procedure was completed with another rx cytology brush.There were no patient complications reported as a result of this event.The patient's condition at the conclusion of the procedure was reported to be good.

The complainant was unable to report the lot number; therefore the manufacture date and expiration date are unknown.However, the complainant stated that the device was used prior to the expiration date.(b)(4).Although the suspect device has been received, the evaluation has not been completed.Therefore, the cause of the reported malfunction has not been determined.Upon completion of the failure analysis of the complaint device, if there is any further relevant information from that review, a supplemental mdr will be filed.

Visual analysis of the returned rx cytology brush found the working length including catheter and pull wire, was bent in multiple places and kinked at 62.5 cm from the heat shrink.Assessment of pull wire at the brush bristle section found the wire had been bent at 1.3 cm from distal tip.The pull wire distal end lumen had been torn for about 2 mm.The brush was bent, functional evaluation found it would fully extend without issue and could not retract into the catheter because of the bent wire.It is possible that the complaint device pull wire was bent due to some aspect of device handling, possibly during shipping, storage, unpacking, or preparation.Therefore, the most probable root cause for this event is determined to be handling damage.
