Event Description
|
It was reported that during use of the device for a cardiopulmonary bypass procedure, the blood parameter monitor (bpm) shut off during case.The perfusionist (ccp) said it smelled a little hot before it shut off.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.

Correction: the device was not changed out.Per the clinical review 08/17/2015: during a cardiopulmonary bypass (cpb) case, a blood parameter monitor (bpm) shut off.The perfusionist (ccp) stated that it smelled a little hot before it shut off.There was no visible smoke or fire.As a result, the ccp turned the power off and continued the case without a bpm.Additional blood gases where required.The case was completed successfully, without delay and without associated blood loss.There was no harm observed.

(b)(4).The reported complaint was confirmed.During laboratory analysis, the product surveillance technician (pst) found that the monitor would re-start while trying to complete the startup self-diagnostics.The auxilliary (aux) printed circuit board assembly (pcba) had an opened/burnt capacitor c59 disrupting the signal in the circuit.The product will be sent to service to be brought to manufacturers specifications before being returned to the customer.No additional action will be taken at this time.
