Sorin group received a report that the touch screen of the s5 double head pump was unresponsive during setup.There was no report of patient injury.

|
There have been two field actions for this issue.The z-numbers are z-0956/0965-2011 and z-1149/1158-2012.(b)(4) manufactures the s5 double head pump.The event occurred in (b)(6).This medwatch report is being filed on behalf of livanova (b)(4).A livanova field service representative was dispatched to the facility to investigate.The service representative was unable to reproduce the reported issue and did not identify any signs of liquid ingress into the pump panel or the touch screen.The touch screen, hkr board and ribbon cables were replaced to resolve the issue.The pump software was then updated to rev 16 per the user-specified parameters.Subsequent functional testing did not identify further issues and the unit was returned to service.The date code of the kapton-tail connection of the replaced touch screen suggests that the root cause is aging and wear of the device and glue.This is a known issue and a root cause investigation (b)(4) has already been investigated.A review of the dhr did not identify any deviations or non-conformities relevant to the reported issue.
