It was identified via the manufacturer's case manager when attempting to follow-up on the patient that there was an obituary online for this patient.It was identified that this patient passed away on (b)(6) 2014.The funeral home was unable to provide information on product disposition or cause of death.The last known treating vns physician for the patient has since retired.A copy of the patient's death certificate was unable to be obtained for the cause of death.Attempts for additional relevant information have been unsuccessful to date.
