Event Description
|
The surgery center reported that the i/v pole bottle hanger was broken and it was found to have the fulcrum pin missing with the whitestar signature system.Additional communication on (b)(6) 2015 verified that the date of the event was unknown because the machine was moved from another location and during installation at its current site, the fss found the problem because tape had been placed on the machine to hold the bottle hanger in position.There was no patient involvement reported and no patient treatments delayed or cancelled.
